Music Monday--Trivia and a Song

Well, I am still having interesting ideas, and I thought I would offer a few music trivia questions for you to answer.  And it is to your advantage to fill it out.  Only I will see it.  All these question except the last one were from my own musical knowledge, and hopefully you can research the answers if you like.  Here's how it will work.

If you even attempt the form, you will automatically receive one additional giveaway entry for giveaways that are current and future ones until the end of this year.  So it is to your benefit to fill out the form--even if all you do is fill out your name and e-mail address!

Now, for each question you get right, you get one more entry into each of the giveaways between now and the end of the year. You will also notice a question at the end that can earn you up to five more entries.  So, do it!!  I will remind you at each giveaway to fill out the form if you have not filled it out.  Don't cheat and try to fill it out more than once because it may cost you those extra entries!  And you never know what giveaway may come along.  I have some more ready to list, but hopefully I will even get more.
